6:45 P.M. Banquet Doors open – Atrium Ballroom (Seating previously assigned)
7:00 P.M. Exemplification Banquet – Atrium Ballroom

8:00 A.M. – 9:00 A.M. Registration – Prefunction Area
9:00 A.M. Candidates report to District Marshal in Salon A
9:45 A.M. Sir Knights assemble
10:00 A.M. Exemplification begins - Salon B & C
10:30 A.M. Ladies Social & Meal - Crystal Room

FOURTH DEGREE MEMBERSHIP APPLICATION:
Applications (Form 4) may be obtained from any Assembly, Council, or the Master of the Indiana
District. An application must be completed in full and signed by the following: Candidate, Proposer,
Faithful Navigator, Faithful Comptroller and the Council Financial Secretary. Failure to obtain all
necessary signatures will delay the processing of the application. IMPORTANT: The application should
be attached to the reservation form provided, along with a check payable to:

NOTE ** Mass will be celebrated by The Most Reverend Gerald Gettlefinger, D.D., Bishop of
Evansville who has given us a dispensation to allow this 2 PM Mass to count for our Sunday Obligation

DEADLINE FOR SUBMITTING APPLICATION/REGISTRATION FORMS IS:
SEPTEMBER 15, 2006

RESERVATION FORM:
Reservation forms should be submitted by every Candidate or Sir Knight attending this Exemplification. This will help us to adequately plan all the functions for the weekend. A reservation form is attached to this notice. Please reproduce additional copies if needed.

EXEMPLIFICATION FEE:
The fee, set by the Supreme Council, for Fourth Degree Candidates is $70.00. This fee includes the following: a social baldric, lapel pin and one dinner ticket. The fee for members of the clergy is $25.00. It is suggested that local assemblies assume the cost for their clergy members.

ATTIRE:
Candidates and Sir Knights will dress in full formal attire, which includes a black tuxedo, black tie, black
cummerbund, black socks, plain white pleated front shirt with a lay-down collar (no frills or colors) and white gloves. This attire is required at the Mass on Saturday and the Exemplification on Sunday. When
taking the Eucharist at Mass, please do not take the host directly into your gloves. The gloves will not be
required at the banquet on Saturday night. Clergy are properly attired in their normal black suit. Military
personnel will be considered in proper attire if dressed in his military uniform. Ladies are requested to wear semi-formal to formal attire to the banquet and a nice dress or dress slacks to the ladies’ activity on Sunday. Dignitaries may wear the regalia of their office or past office during the Mass and exemplification ceremony. This would include Faithful Navigators, Past Faithful Navigators, Assembly Commanders, Masters, Former Masters, Vice Supreme Master and Former Vice Supreme Masters.

HOTEL ACCOMMODATIONS:
A block of rooms has been set aside for this exemplification at the Executive Inn Evansville, 600 Walnut
Street, Evansville, Indiana, 47708-1624. Each individual is responsible for making his own room reservationdirectly with the hotel. Reservations can be made by calling (877) 424-0888. Our group rate is $75.00 + tax per night. DEADLINE FOR ROOM RESERVATIONS IS SEPTEMBER 6, 2006.

PLEASE MENTION THAT YOU ARE WITH THE KNIGHTS OF COLUMBUS WHEN MAKING YOUR RESERVATION. AFTER THE DEADLINE, THE RATE IS BASED UPON AVAILABILITY.

HOSPITALITY ROOMS:
Each assembly must make their own arrangements for any hospitality rooms directly with the hotel.

CLASS PICTURE:
A photograph of the exemplification class, degree team and dignitaries will be taken following the
exemplification ceremony. You will be afforded an opportunity to place an order and pay in advance for the photograph when you register.
